The Australian Drug Foundation (ADF) would like to invite you to take part in an evaluation of our new drug driving website. Your views are very important to us and will help us to meet the needs of people who access the website.

This survey is open to all Australian residents who have visited the ADF's drug driving website (http://www.drugsdriving.adf.org.au/). 

Those who complete the survey have the option of entering a draw to win one of five double passes to Hoyts Cinemas!  (Conditions apply: maximum of one entry per person; employees of the ADF are not eligible to enter).  To enter the draw respondents simply have to provide their email address at the end of the survey.  Please note that respondents' email addresses will not be stored with their data.  Email addresses will be electronically separated before the data is viewed, and connections between email addresses and the data will not be able to be made.  The raffle will be drawn August 29, 2008.

The survey should take not longer than 10 minutes to complete.

This survey does ask about personal drug use and drug driving behaviour.  However all surveys are confidential and anonymous. No individuals will be identified in the report of the findings (produced for internal use only).  Completion of the survey means that you consent to participate. Data will be securely stored at the ADF and access will be limited to ADF research staff. You will not be contacted again as a result of completing this survey.

• All open-ended questions are optional

• If you wish to see a previous page you will need to use the ‘prev’ button at the bottom of the page rather than the back arrow of your web browser (your web browser arrows will take you out of the survey altogether)
